Medical Marijuana for Chronic Pain in Washington State

Chronic pain is the most frequently cited qualifying condition in Washington State's medical marijuana program — and for good reason. For patients who haven't found adequate relief through conventional treatments, cannabis offers a different mechanism of action that many find genuinely helpful.

What counts as chronic pain in WA?

Washington State does not require a specific diagnosis for chronic pain to qualify. The state's statute recognizes chronic pain as a qualifying condition when it is caused by a medical condition, injury, or disease and has persisted for three months or more. This is intentionally broad — it can include:

  • Back pain and spinal conditions
  • Neuropathic (nerve) pain
  • Fibromyalgia
  • Arthritis and joint pain
  • Migraine and chronic headache disorders
  • Post-surgical pain
  • Cancer-related pain
  • Musculoskeletal pain from injury or overuse

If your pain significantly impacts your daily functioning and has lasted more than three months, you likely qualify. Your provider will assess your specific situation.

How does cannabis help with chronic pain?

Cannabis interacts with the endocannabinoid system (ECS), which plays a key role in pain modulation. Both THC and CBD have demonstrated analgesic properties through different mechanisms:

  • THC binds to CB1 receptors in the brain and nervous system, reducing pain signal transmission and altering pain perception
  • CBD has anti-inflammatory effects and modulates pain through non-CB1 pathways, including serotonin and vanilloid receptors
  • Combined (the "entourage effect"), these compounds may be more effective together than in isolation

Research on cannabis for chronic pain is more extensive than many realize. A 2022 analysis in JAMA Network Open found that medical cannabis patients reported significant improvements in pain severity and quality of life. For many patients, cannabis also reduces dependence on opioids and NSAIDs.
