Why records are required
Washington's RCW 69.51A authorization is issued when a licensed healthcare provider determines you have a qualifying condition. At Green Wellness, that determination is made by reviewing documentation of your condition — a diagnosis letter, chart notes, imaging, lab results, or medication history from the provider who treats you — together with your history and an examination. Records protect you, too: an authorization backed by documented medical history stands up when a dispensary, employer, or the DOH looks at it.
What you don't need
- A referral — no doctor has to send you to us; you book directly
- A specific specialist — records from any treating provider (primary care, specialist, therapist) documenting your condition work
- Perfect paperwork — recent chart notes or a visit summary mentioning your condition are usually enough; roughly the last 1–3 years is ideal

What happens at the evaluation
Your provider reviews your records and asks about your condition, how long you've had it, how it affects your daily life, what treatments you've tried, and your current medications. If your documented condition qualifies and your provider determines medical cannabis is appropriate, you'll receive your written authorization the same day — valid at all licensed Washington dispensaries, with optional $1 DOH registration for the full tax exemption.
